Fees and Insurance
At this time the only insurance network I am a provider for is ASHN (American Specialty Health Network). Many compaines use ASH, so please check your insurance card.
If you have ASH coverage through Kaiser, you will need a referral though your primary care doctor before they will pay for acupuncture.
My cash fees are:
Initial Visit - Pain, Morning Sickness, Turn Breach Baby, IVF pre, or Post Transfer-$100
Initial Visit - All other $125
Follow up visits - $75
These fees do not include herbs or supplements
New Patient Forms and Resources
If you are a new patient Please print out and bring in the appropriate forms (below) filled out prior to your visit. If you cannot print them, please let us know, and we can arrange to have them mailed. For
ASH Patients please fill out the ASH Forms as well.
